Perioperative Considerations for Patients Treated with Bleomycin (5)Hyaline membranes have also been described in some patients. Pulmonary lesions show no particular preference for any portion of the lung, and lesions of all stages may be present in a single lung. These lesions consist of squamous metaplasia of the bronchiolar epithelium, muscular hyperplasia of the bronchioles, hyperplasia of type 2 epithelial cells, aggregations of alveolar macrophages, and the development of collagen fibers in the septae.” Further progression of these histopathologic changes can result in a typical pattern of fibrosing alveolitis or “honeycomb lung.” Progression of fibrotic changes has been shown to occur up to 12 weeks after treatment in mice, but there are no long-term studies in the literature which indicate resolution after damage has occurred. ventolin 100 mcg
Structural changes at the cellular level result in functional changes in the lung as well. Studies in animals have shown that bleomycin-induced and hy-peroxia-induced damage of type 2 epithelial cells resulted in changes in the phospholipid composition of pulmonary surfactant and in the amount of surfactant produced. These findings may be relevant to the study of surfactant replacement therapy in humans.
