Natural history of C282Y homozygotes for hemochromatosis: DISCUSSION Part 2

In: Hemochromatosis

DISCUSSIONSince our study in 1991, the number of asymptomatic and younger patients with hereditary hemochromatosis has increased. C282Y homozygotes with a serum ferritin concentration less than 1000 pg/L and normal aspartate aminotransferase levels, and without hepatomegaly were not biopsied in the present study. Previous studies from our centre London Health Science Centre and from France have demonstrated that cirrhosis is not present in this subgroup of patients. Despite earlier diagnosis, life-threatening complications were found in 36% of men and 19% of women in the present study. These complication rates are likely an overestimation due to referral bias, and the percentages decrease with time as more asymptomatic patients are evaluated; however, the rates are similar to those reported in family members of affected probands. The percentage of life-threatening symptoms has been demonstrated to be an important variable in the economic analysis of population screening for hemochromatosis because the cost of undiagnosed and untreated cases is high if there is significant morbidity and mortality.
